Failure to format S6,D2 or D1 with System Utilities

I created a new image with CFFA3000 v3.1 then loaded that into virtual S6,D2.
In virtual S6,D1 I loaded System Utilities v3.2 and booted S6.
I chose System Utilities and then tried to format S6,D2. It fails each time I try it.
I would expect this to work, given that it is software provided by Apple.

The debug log shows a write error during this process at D2, T2

System Utilities v3.2 obtained from
ftp.apple.asimov.net/pub/apple_II/images/masters/prodos/Apple II System Utilities v3.2 (ProDOS8 v1.9 1990).dsk

I would have uploaded the log as an attachment however every (non/)extension I tried (.log / .txt) was 'not allowed' - huh?

CFFA3000 is in Slot 7 of a Platinum //e International version. No dips are set. USB attached. No CF attached. No card in Slot 6. No accelerator present.

I also tried booting the System Utilities in Slot 7 and trying to format the blank image in S6,D1. I got the same result.

Ok, I should have RTFM...

Formatting scenarios that are known to NOT work are:
- Formatting performed by the ProDOS system utilities
- Formatting performed by the Apple Pascal Operating System
